Community Health Partners, a private nonprofit organization,
began in 1995 through a collaborative effort between Orange City
Municipal and Sioux Center Community Hospitals.
The organization, Home Health Resources was created to
provide management services for the hospitals' home health programs.
Home Health Resources added a third hospital member in 1996,
Hegg Memorial of Rock Valley. In 1997, the Home Health Resources
Board of Directors entered into an agreement with the
Sioux County Board of Health for delivery of
the county public health services. The organizations
name was changed to Community Health Partners.
Northwest Iowa Health Center was added
as a fourth hospital partner in 1998.
Today, in addition to the hospital partner members,
community and board organizations pursuing the ongoing mission of -
"Building Healthy Communities"
